GENERAL TERMS

1.Veteran agrees that this Service Dog will be given adequate and proper shelter and housed and exercised in a
fenced, secure yard or dog park. This service dog shall not be neglected, allowed to roam free, or be mistreated in any
manner. The service dog shall receive proper veterinary care and inoculations throughout its life and placed on a
heartworm preventative if needed in their state. The Service Dog shall receive adequate nutrition to maintain proper
weight. The veteran agrees that the Service Dog will be with him at all times once proper papers are attained.

2.The Veteran will notify Operation Wolfhound of any changes of address and will maintain contact with the Operation
Wolfhound throughout the life of this dog. The Veteran will notify Operation Wolfhound of any AKC, CKC or other titles
earned by this Service Dog.

TRAINING PROSPECT TERMS

Hereinafter referred to as "Service Dog" has been placed with Veteran by Operation Wolfhound after screening and
donation by the Breeder as a“Service Prospect" Service Dog or adult.

Service Prospect shall be understood to mean that the animal has been prescreened for temperament and working
ability and further that no genetic disorders are immediately apparent.

Should Service Dog develop any major faults preventing it from working with the veteran properly or the veteran’s
circumstances change thereby causing Veteran and Operation Wolfhound representative agree that the animal cannot
perform its intended purpose the Veteran will be provided with one of the options below.

2. If there is a conflict between the Operation Wolfhound representative and the Veteran as to the merit of the Service
Dog, the decision will rest with a mutually agreed upon alternate representative, professional trainer, or Borzoi breeder.

    a) Should Veteran wish to return Service Dog for another service prospect replacement, Veteran will pay for
    expenses to return Service Dog and Operation Wolfhound will provide replacement Service Dog as soon as such
    a Service Dog is available.

3.Breeder and Operation Wolfhound shall not be held responsible for the development of disqualifying faults, diseases,
or disorders, which are due to Veteran’s negligence.

4.The Veteran agrees to engage in a minimum of six months and up to two years of intensive training with their Service
Dog, in order to meet the requirements for an IAADP registered service dog. Training requirements will be outlined and
agreed upon by Veteran and Operation Wolfhound trainer before placement.

If the Veteran and donating Breeder wish to engage in AKC competition with the Service prospect Service Dog, the
veteran will work with and follow the AKC breeder's reasonable suggestions for showing and competition.

5.Veteran agrees that Service Dog shall not be used for breeding purposes and will be altered. The ONLY exception to
this rule is if the Breeder of the dog, before donation, offered and designated the dog to be offered on a co-ownership
for a veteran interested in AKC or Field competition. Otherwise the dog becomes the sole property of the veteran.

Additionally to general breeder agreement on the co-owned Service Animal,unless the following certifications have been
received by the Veteran's dog prior to breeding, the breeding will not be allowed or certified:

    Heart Test by Cardiologist
    Written Permission by Breeder who donated Service Dog
    OFA Thyroid normal certification
    The Canine Eye Registration Foundation (CERF) within 12 months of the planned breeding.


CO-OWNERSHIP TERMS

1. Veteran agrees that Service Dog will be given on a Co-Ownership
contract in which Veteran will be the first registered owner on AKC paperwork and Breeder the second owner listed.

2. Veteran will have Service Dog micro-chipped listing the Breeder as second contact. IAADP participation and
registration provides both the chipping and registration with multiple services for free and the veteran must use this
service.

3. If the Service Dog cannot be kept by the Veteran, the Veteran shall notify Operation Wolfhound immediately. The
Service Dog shall not be sold or given to any individual or to any retail or wholesale establishment for the purpose of
sale or surrender to any rescue organization, animal shelter or humane society. The Service Animal must be returned to
Operation Wolfhound.

4. Upon return of Service Dog, the Veteran agrees to sign the necessary AKC papers to list Operation Wolfhound as
sole owner of the dog.

5. Veteran agrees the service dog will be neutered or spayed. Unless non-alteration is negotiated with the donating
breeder in advance of placement.

6. The Veteran will obtain a letter from a councilor stating his need for a psychiatric service animal. This letter will then
be used to file the Service Dog paperwork with the VA recognizing the Service Dog as the veteran’s service animal in
training. If the VA approves; Service Dog will be eligible for government funding for Service Dog’s care and upkeep,
including catastrophic coverage of veterinarian bills.

7. The Veteran agrees to provide the Service Dog with a quality dog food such as Kirkland, Diamond, Iams, Bil-Jac,
Science Diet or equivalent quality brands. If the Veteran wishes to feed Biologically Appropriate Raw Foods, the Veteran
must ensure Service Dog has adequate and balanced nutrition. It is also understood that Old Roy and other similar
brands are not considered a quality, adequate food.


HEALTH GUARANTEE

1. Operation Wolfhound guarantees that the dog is in good health when placed and has received appropriate medical
inoculations, worming and care. Breeder will provide Operation Wolfhound with a medical chart detailing treatment the
dog has received under Breeder’scare, and any future care Breeder feels is essential to the dog's well being.


Should the Veteran be unable or unwilling to fulfill any of the above provisions, the Veteran agrees to return the dog to
Operation Wolfhound at Operation Wolfhound's request. Should the Veteran refuse to return the dog, the Veteran
hereby authorizes Operation Wolfhound to pick up and remove the service dog from the Veterans' control.
The Veteran agrees to pay Operation Wolfhound any and all expenses, including court costs and reasonable attorney
fees, in enforcing the terms and provisions of the above contract.

This Agreement will be governed by the laws of the State of______________and may not be altered, modified or
rescinded unless agreed to in writing by Veteran and Operation Wolfhound. The Veteran agrees that any legal
proceedings will take place in the state of__________________.

The undersigned agree to uphold all covenants of this Contract.
